Allay Hospice Mission
Allay Hospice will consistently strive to provide you and your family with the best compassionate care and comfort that can be experienced through the last phase of one’s own life journey.
Our Team of dedicated professionals is committed to treating you and your family as we would care for our own. Our Nurses, Aides, Chaplains, Social Workers and other staff will complement your current family team in providing care, comfort and support while still allowing you to be the decision maker regarding your treatment.
Your Allay Hospice Team
Staff & Services offered
At Allay Hospice we believe in providing care with a “team approach.” All of the following specialties will share in the care and coordination of you or your loved one’s care. The following outline gives you an idea of who the team members are and what their primary role is:
The Hospice Physician directs the medical care and works in conjunction with your attending physician and the Allay Hospice team.
Your Attending Physician
Your doctor will play an important role in your care. The Case Manager who you were assigned will be calling your doctor to coordinate all areas of your care. Your doctor will continue to authorize medications, treatment and other related functions as indicated in your Plan of Care.
Our Social Workers are a resource for supporting any emotional issue you or your loved one might be experiencing relating to your health condition. Our Social Workers will listen and help formulate responses to those complicated situations or feelings that arise. The Social Worker is also a contact for community resources, & assistance with placement or other services if needed
Allay Hospice Chaplains will provide spiritual & emotional support to you and your family while in our care. Many patients and their families feel spiritual support is just as important as their medical treatment. Support is offered to all people, regardless of faith tradition or spiritual beliefs. Our Chaplain will communicate and work together with you and your current clergy.
Our Hospice Nurses specialize in caring for patients with life-limiting illnesses. Skilled nursing care including providing symptom management, education & emotional support.
Our Bereavement Coordinator will support you and your loved one for up to 13 months after your loved one’s passing. Phone, face to face contacts, mailings and group meetings are organized in order to help facilitate hope and support for you.
Our On-Call Nurse is available 24 hours a day by phone to answer any questions you might have. If it is necessary our On-Call Nurse will visit you in order to help resolve any discomfort or problems you are experiencing.
Our Volunteers are trained caring men and women that want to help and contribute to you and your loved ones care. Volunteers will help you with errands, companionship and other opportunities to be of service.
Home Health Aides
Our Aides will assist the patient with any personal care activities including bathing. They also help with light meals or basic hygiene activities.
Other Services – Therapy
Among the many services offered by our team at Allay Hospice we also recognize the frequent need for other therapy such as Physical Therapy, Occupational Therapy and Speech Therapy. The goal would be for comfort & quality of life for the hospice patient.
