Home health care can help patients avoid unnecessary hospitalizations, speed up recovery time, and allow them to stay in their own homes where they can feel safe and secure.

Allay Home and Hospice, we understand that each patient has unique needs. We also understand there is no “one-size-fits-all” solution when it comes to healthcare. Home Care is a great option for those who need extra assistance, but do not need to be in a hospital or other care facility. Our services can help patients avoid unnecessary hospitalizations, speed up recovery time, and allow them to stay in their own homes where they can feel safe and secure.

What are the Benefits of Home Care services?

The Home Health option allows patients to recover from an illness in the comfort of their own home. The patient can remain independent, continue their daily routine, and recover with a low level of stress. We want patients and loved ones to be confident in the care they receive so we evaluate their homes during the initial assessment to ensure there are not any hazards.

 

  • Each care plan will be unique and specially tailored to fit a patient’s needs
  • We will provide a clear explanation of each part of the care plan
  • Physicians, nurses, therapists, and other members of the care team will work closely together and adjust care plans as necessary
  • We will place the emotional and physical well-being of the patient as our first priority


Who is eligible for home health?

 

Patients may be eligible if:

 

  • Patients require additional assistance due to illness, disability or aging. 
  • There is a physician’s order for skilled home health services
  • The patient is homebound following an injury or surgery
  • Patients who need physical, occupational, or speech therapy due to an injury or surgery.
  • Patients with conditions such as:
  • Fall Risk
  • After Surgery Care
  • Wound Care
  • Heart Disease
  • Respiratory Disorders
  • Diabetes
  • Medical Management & Teaching
  • Post Rehabilitation

The majority of home health care focuses on:

  • Helping patients recover from an illness, injury, or surgery
  • Providing patient and caregiver education
  • Increasing independence or managing a chronic, ongoing illness (such as heart disease, pulmonary disease, or kidney disease)

The Allay Home Care Team

At Allay Home Care, we believe in providing care with a “team approach.” All of the following specialties will share in the care and coordination of you or your loved one’s care. The following outline gives you an idea of who the team members are and what their primary role is:

Your Attending Physician

Your doctor will play an important role in your care. The case manager who you were assigned will be calling your doctor to coordinate all areas of your care. Your doctor will continue to authorize medications, treatment and other related functions as indicated in your plan of care.

Home Health Aide

Our Aides will assist the patient with any personal care activities or bathing. They also help with light meals or basic hygiene.

Social Worker

Our Social Workers are a valuable resource to patients and families receiving Home Care services. They can provide support and connect patients with resources in the community to assist in the caregiving and well-being of the family continuing on after the Home Care services are complete.

Nursing Staff

Our Home Care Nurses specialize in caring for patients with many types of conditions, chronic disease management, wound care, medication management, & training as well as many other skilled nursing services.

On-Call Nurses

Our On-Call Nurses are available 24 hours a day by phone to answer any questions you might have. If deemed necessary, a visit will occur to help resolve any discomfort or problems you are experiencing.

Medical Director

Our Home Care Medical Director meets with our Home Care team to review and consult on the progress of our home care patients every two weeks. This review is not required by Medicare but is a reflection of Allay’s commitment to excellence in patient care.

Director of Clinical Services

The Home Care Director of Clinical Services manages the clinical staff of the Allay Home Care team.

Therapy Services

Among the many services offered by our team at Allay Home Care therapy needs such as Physical Therapy, Occupational Therapy and Speech Therapy can be met. Our goal is to return the patient to the highest possible function as soon as possible.

Interdisciplinary Team

The core of our service involves you, your family or caregiver, your attending physician, the Home Care Medical Director, Nurse, Social Worker, Home Health Aide, & therapy. Together we will formulate a Plan of Care specific to you and your loved ones needs.
