Allay Home Care will consistently strive to provide you and your family with the highest quality personalized care both medically and emotionally with the utmost compassion, We will address and treat your specific needs in order to help you return to your everyday activities as soon as possible.
Our Team of dedicated professionals is committed to treating you and your family as we would care for our own. Our Nurses, Aides, Social Workers, Physical, Occupational and Speech Therapists and other staff will compliment your current family team in providing care, comfort and support while still allowing you to be the decision maker regarding your treatment.
Allay Home Care is a Medicare Certified Part A provider. There is no co-pay or cost to the patient while receiving our Home care.
To be eligible for Allay’s Home Care Services patients must be home bound, they must require a need for skilled healthcare, and they must acquire an MD’s order for treatment. The Allay Home Care team will conduct an evaluation initially to determine your eligibility. Your attending Physician and our Home Care Team will then provide the needed services to help you return to your regular activities as quickly as possible.

Your Allay Home Care Team
Staff & Services offered
At Allay Home Care we believe in providing care with a “team approach.” All of the following specialties will share in the care and coordination of you or your loved one’s care. The following outline gives you an idea of who the team members are and what their primary role is:
Your Attending Physician
Your doctor will play an important role in your care. The case manager who you were assigned will be calling your doctor to coordinate all areas of your care. Your doctor will continue to authorize medications, treatment and other related functions as indicated in your Plan of Care.
Home Health Aide
Our Aides will assist the patient with any personal care activities or bathing. They also help with light meals or basic hygiene.
Our Master's level Social Workers are a valuable resource to patients and families receiving Home Care services. They can provide support and connect patients with resources in the community to assist in the care-giving and wellbeing of the family continuing on after the Home Care services are complete.
Our Home Care Nurses specialize in caring for patients with many types of conditions, chronic disease management, wound care, medication management, & training as well as many other skilled nursing services.
Our On-Call Nurse is available 24 hours a day by phone to answer any questions you might have. If it is necessary, our On-Call Nurse will visit you in order to help resolve any discomfort or problems you are experiencing.
Our Home Care Medical Director meets with our Home Care team to review and consult on the progress of our home care patients every two weeks. This review is not required by Medicare but is a reflection of Allay's commitment to excellence in patient care.
Director of Clinical Services
The Home Care Director of Clinical Services manages the clinical staff of the Allay Home Care team.
Among the many services offered by our team at Allay Home Care therapy needs such as Physical Therapy, Occupational Therapy and Speech Therapy can be met. Our goal is to return the patient to the highest possible function as soon as possible.
The core of our service involves you, your family or caregiver, your attending physician, the Home Care Medical Director, Nurse, Social Worker, Home Health Aide, & therapy. Together we will formulate a Plan of Care specific to you and your loved ones needs.
